Support for health and everyday life

Our planned services combine community nursing with practical support at home and in the community.

We discuss your needs and goals before agreeing on support. Services will depend on suitability, staff capability and availability.

Community Nursing

We are preparing to offer nursing support in the home. Your needs will be assessed before agreeing on the care to be provided, who will deliver it and how care will be reviewed.

Ask us about the nursing support you are looking for. We will confirm whether it falls within our available service scope before accepting a referral.

Household Assistance

Planned assistance includes agreed cleaning tasks and basic gardening or lawn care. Tasks, visit arrangements and any exclusions will be discussed before services begin.

Major landscaping, trade work and hazardous tasks are outside the proposed everyday assistance offer.

Community Access and Appointment Support

Support with attending appointments and participating in everyday activities will be planned around your preferences and agreed needs.

Any transport arrangements and related costs must be confirmed in advance. This is not ambulance or medical transport.

Fees and funding

Before services commence, we will explain the agreed costs, including any applicable travel and cancellation arrangements. Funding suitability will be discussed for your circumstances; not every task is automatically funded through an NDIS plan.

Service areas and availability

Our planned areas are Melton, Brimbank, Wyndham and Hume, with Bacchus Marsh enquiries welcome. Exact coverage and service availability will be confirmed before an arrangement is made.
